New Patient Registration

If you live within the practice catchment area (includes all N5 and a number of N4 postcodes) and fulfil the registration criteria you are able to register as a regular patient.

The Registration Criteria are as followed:

  • We welcome registration from all new patients.
  • Patients who live outside our practice area are welcome to register, but we are unable to provide home visits for patients living outside our boundary.
  • NHS Number – patient must obtain number prior to registration appointment except where this is the first NHS registration

Catchment Area

If you are registering as a regular patient you will be required to make an appointment for a new patient health check with either the health care assistant or practice nurse.

To register please complete the registration form and a health questionnaire (online versions available below) or you can provide reception with a NHS medical card, if you have one.

Please notice that we will ask for your postcode to check if you are in the practice’s catchment area. We are sorry if you are outside the area as it means we cannot register you, but you can find further information here about where you may be able to register. 

Accepted Documents for Proof of Residency

  • Rent or Tenancy Agreement
  • Council Tax Bill – Must be valid in current year
  • Mortgage Statement
  • Confimation letter from Council regarding Electoral Register
  • Social Security Letter

For Disability Access;

There is a ramp leading to the entrance doors, automatic sliding doors at the main entrance, and no internal steps within the premises. Disabled toilets are available.
